Patient Appointment Prep
- For all appointments please bring your insurance card and picture ID.
- For vein screenings, vein consultations, and vein procedures – you may wear or bring shorts but be prepared to change if the affected area cannot be accessed.
- For UFE consultations please bring all relevant exam images and reports if possible. If you cannot easily access these, we will have you sign a release form and obtain your records after your appointment.
- For all surgery procedures please have someone come with you to drive you home after the procedure.
A staff member will contact you the day before your appointment to give you specific directions regarding your medications. - Please do not have anything to eat or drink the day of your surgery procedure unless otherwise given clearance.
All patients will receive a reminder phone call the day before their appointment to confirm the appointment time as well as discuss any specific directions.
